How much does Mounjaro cost – and does health insurance pay?

In short: A Mounjaro KwikPen lasts four weeks and, in German pharmacies, costs roughly €207 to €490 depending on strength (as of September 2026). For weight loss, German statutory health insurance does not pay, because weight-regulating drugs are excluded by law – you get a private prescription instead. Mounjaro prices 2026 per KwikPen and month

In Germany, Mounjaro (active ingredient tirzepatide) comes as a KwikPen with four weekly doses. So one pen covers roughly a month. The price rises with the strength. Prescription-only medicines in Germany have a uniform pharmacy retail price, so prices hardly differ between pharmacies.

For context: Lilly changed its list prices in Germany in February 2025. The 2.5 mg starting dose became cheaper, the higher strengths considerably more expensive. Does health insurance pay? GKV, PKV and Beihilfe

Statutory health insurance (GKV), goal weight loss: No. § 34 SGB V excludes medicines whose main purpose is improving quality of life – explicitly including drugs for slimming, appetite suppression or regulating body weight. This applies even if you have severe obesity with comorbidities.

GKV, type 2 diabetes: If Mounjaro is prescribed to treat type 2 diabetes that is not adequately controlled, you get a statutory prescription. You then only pay the usual co-payment of at most €10 per pack. For diabetes use, Lilly agreed a confidential reimbursement price with the German health insurers in 2025.

Are there changes in 2026? Not yet. The Bundestag’s Petitions Committee has forwarded a petition on cost coverage to the Federal Ministry of Health, because new scientific findings justify a review. In 2026, the chair of the G-BA (Federal Joint Committee) signalled that reimbursement for clearly defined patient groups would be conceivable – but only if lawmakers change the law. As of September 2026, no such change in the law has been made.

Private health insurance (PKV): Here it depends on your plan and medical necessity. Typical requirements are a BMI of 30 or more (or 27 or more with comorbidities such as high blood pressure or prediabetes), a doctor’s justification and documented unsuccessful attempts with diet and exercise. Ideally, clarify reimbursement in writing before starting treatment. If you are turned down, you can appeal.

Beihilfe (civil servants’ allowance): Many Beihilfe and subsidy offices follow the lifestyle exclusion. The health scheme for federal railway civil servants (KVB), for example, does not subsidise drugs “for slimming, appetite suppression or regulating body weight”, but it does cover Mounjaro for type 2 diabetes. The federal government and the states have their own rules – ask your Beihilfe office.

Statutory or private prescription – the difference

  • Statutory prescription (pink): only for type 2 diabetes. The insurer pays, you pay the co-payment.
  • Private prescription (blue or white): when used for weight loss. You pay the full pharmacy price yourself – even if you have statutory insurance.
  • E-prescription: Both types also exist in digital form. You can redeem them at any pharmacy, including licensed online pharmacies.

Tip: Keep your pharmacy receipts – you will need them if you submit a claim to PKV or Beihilfe.

How to get a prescription

Mounjaro is prescription-only. According to the prescribing information (SmPC), it is approved for weight management in adults with a BMI of 30 or more, or a BMI of 27 or more plus at least one weight-related comorbidity – always in addition to fewer calories and more exercise.

  1. GP practice: The first port of call for most people. Blood pressure, blood sugar (HbA1c), kidney values and pre-existing conditions are checked there.
  2. Diabetology or obesity clinic: Makes sense if you have diabetes, severe obesity or several comorbidities.
  3. Telemedicine: Online doctors’ practices also issue private prescriptions. In 2026, the German consumer advice centres (Verbraucherzentralen) criticised that some platforms check neither identity nor weight. An offer is only trustworthy if real information is requested and a doctor takes responsibility.

Tips for talking to your doctor

  • Bring current values: weight, height, waist circumference, blood pressure and – if available – lab results.
  • Mention all medications, including blood pressure drugs, the pill and supplements. See interactions.
  • Explain what you have tried so far (diet, exercise, programmes). This often matters for PKV and Beihilfe.
  • Ask specifically: What is the target dose, how often will we check in, and what happens when stopping?
  • Talk openly about the cost. A plan for the maintenance dose helps you estimate the expenses realistically.

Mounjaro vs. Wegovy: price comparison and “research tirzepatide”

In Germany, Wegovy (semaglutide) costs about €170 to €280 per pen depending on strength, which also contains four weekly doses (as of September 2026). On the maintenance dose, Wegovy is therefore usually cheaper than high-dose Mounjaro. Which medication suits you better does not depend on price alone, though – see Mounjaro vs. Wegovy.

Because of the high cost, some people switch to unregulated “research” tirzepatide powders that are sold as research chemicals “not for human use”.

Why this is risky: These products are not approved medicines. Nobody checks purity, content, sterility or contamination, and if problems arise you have no medical supervision. If the price is the problem, it is better to talk openly with your doctor about it – for example about whether a lower maintenance dose might be enough for you.

When to see a doctor

  • You have type 2 diabetes and have been paying for Mounjaro privately – have it checked whether a statutory prescription is possible.
  • You take insulin or sulfonylureas: according to the prescribing information (SmPC), the dose of these drugs may need to be reduced to avoid hypoglycaemia – your doctor will clarify this before you start.
  • You have pre-existing conditions such as pancreatitis, gallbladder problems or kidney disease.
  • You have used Mounjaro without medical supervision or an unregulated product and develop severe abdominal pain, vomiting or circulatory problems.
