How much muscle mass is lost?
In a substudy of SURMOUNT-1, body composition was measured by DXA: on tirzepatide, considerably more fat than lean mass was lost, roughly in a 3 to 1 ratio. That matches what is seen with diets without medication – so the proportion is not unusually high. But because the total loss is large, it pays to actively counteract it.
Important: “lean mass” is not just muscle, but also water, organs and bone.
Protein: how much makes sense?
For people losing weight, professional societies and studies often cite about 1.2–1.6 g of protein per kilogram of body weight per day; with severe overweight, the target weight is often used as the reference. Because appetite is low on GLP-1 medications, it helps to eat the protein first at every meal and to spread it across the day.

- Protein shakes or protein coffee are handy when solid food is hard to manage

Strength training: the strongest lever
Muscles stay when they are used. 2–3 strength-training sessions per week – on machines, with weights or with your own body weight – are a good guideline. If you are hardly eating anyway, pay particular attention to protein and fluids on training days.
How can I tell whether I’m losing muscle?
The scale alone tells you little. Waist circumference, your strength numbers in training and body-composition scales (BIA) are more helpful – although the latter fluctuate a lot with your hydration. Most meaningful is a trend over several weeks, always measured under the same conditions (e.g. in the morning before eating or drinking).

- Noticeable loss of strength in daily life (climbing stairs, getting up)
- Persistent weakness, dizziness or a racing heart
- Very rapid weight loss while barely eating
