Nausea on Mounjaro, Wegovy & Co.: What can you do?

In short: Nausea is the most common side effect of tirzepatide and semaglutide – especially in the first weeks and after every dose increase. Small portions, eating slowly and stopping in time help most. Persistent vomiting or severe abdominal pain are a matter for your doctor.

Why do I feel sick?

GLP-1 medications keep food in the stomach longer and act directly on the satiety centres in the brain. If you eat the way you did before, you quickly overfill your stomach – the result is nausea, burping or a feeling of fullness.

In the SURMOUNT-1 approval study, depending on the dose, about a quarter to a third of participants on tirzepatide reported nausea, compared with just under 10% on placebo. It was mostly mild to moderate and eased after a few weeks.

9 tips against nausea

  • Small portions – better 4–5 small meals than 3 large ones
  • Eat slowly and stop as soon as you are full, not only when your plate is empty
  • Avoid fatty, fried and very sweet foods – they sit in the stomach especially long
  • Don’t lie down straight after eating
  • Drink throughout the day, but not large amounts right with your meals
  • Cut down on fizzy drinks and alcohol
  • Ginger (tea, fresh) helps some people
  • Eat protein first – it is filling and usually well tolerated
  • Some people tolerate the injection better in the evening because the strongest phase falls during sleep – discuss the timing with your doctor

And dose increases?

Nausea is strongest after each increase. If it significantly limits you, talk to your doctor about whether you should stay on the current step for longer. Don’t change the dose on your own.

When to see a doctor

  • You can’t keep anything down for more than a day
  • Signs of dehydration: very little urine, dizziness, dry mouth
  • Severe abdominal pain, especially in the upper abdomen radiating to the back
  • Pain in the right upper abdomen, fever or yellowing of the skin (gallbladder)

How long does nausea last on Mounjaro?

Usually a few days to a few weeks after starting or a dose increase. For most people it becomes much less over time.

Does it help to inject in the evening?

Some people report that it does, because the strongest phase falls during sleep. There are hardly any studies on this – best to agree the timing with your doctor.

Is nausea a sign that the injection is working?

Not necessarily. The medication works without nausea too. Nausea is more a sign that you ate too much or too fast.
